Case Management

The Honolulu Community Action Program’s (HCAP's) ultimate goal of case management is to promote and support independence and self-sufficiency. Case management is a multi-step process to ensure access to and coordination of social services to meet an individual’s and family’s comprehensive needs.

Case management includes the following processes: intake, assessment of needs, service planning, implementation, monitoring and follow-up, crisis intervention, and case closure.

HCAP’s case management activities are diverse. In addition to assisting clients to access and maintain specific services, case management activities may include, but are not limited to:

  • Employment readiness training and employment search assistance

  • Clothing for interviews and/or work

  • Homeless housing referral assistance, Section 8 referral, housing referral assistance

  • Food security programs

  • Energy and utility assistance programs

  • Assistance in filling out paperwork, such as welfare, insurance, and loan documents

  • Copy machine and fax machine usage

  • Community information and resource center, and one time assistance providing minor items such as hygiene products, soap, toothpaste, toothbrush, toilet paper, etc.

Get a Social Worker to Refer you to the Housing located in the Community Resource Guide or this Housing List. When calling they often say you need to get a referral (from a Social Worker) Here is the phone numbers to be considered homeless meet at your homeless site where you usually are at a certain hour, and they will come and meet you. It's good to also get a Verification letter of Homelessness to get some services like a case manager that is not Mental Health Case Manager Home - Hawai‘i CARES 988 (hawaii.gov)

The provider administering the State Housing First Program in Honolulu is

IHS Institute for Human Services the charity gives out they tell you how you need a referral from partners in care get a case manager call according to where you are in need of help,
